Swiss national guidelines, put in place since the close of 2018, emphasize the need for support of HIV-positive women who opt to breastfeed. We seek to articulate the inspirational drivers prompting these women and their offspring, and to evaluate their results.
Mothers enrolled in the MoCHiV study, delivering between January 2019 and February 2021, meeting the optimal scenario criteria (adherence to cART, regular clinical care, and suppressed HIV plasma viral load (pVL) below 50 RNA copies/ml), and electing to breastfeed following shared decision-making, were invited to participate in this nested study and complete a questionnaire assessing their motivations for breastfeeding.
During the period from January 9th, 2019, to February 7th, 2021, 41 women gave birth; 25 of those new mothers chose breastfeeding, and 20 of them volunteered for the nested study. These women's main motivators were rooted in the desire for social connection, the positive influence on the health of newborns, and the advantages to maternal health. The median duration of breastfeeding was 63 months, ranging from 7 to 257 months, with an interquartile range of 25 to 111 months. The breastfed neonates were not a recipient of HIV post-exposure prophylaxis. No HIV transmission was detected in the twenty-four infants tested; each was found negative for HIV at least three months after weaning, while one mother continued breastfeeding at the time of the study.
From the shared decision-making process, a substantial number of mothers articulated their choice to breastfeed. Infants who were breastfed remained free from HIV infection. Continued observation of breastfeeding mother-infant pairs within high-resource healthcare settings is necessary for modifying and updating recommendations and guidelines.

This retrospective study examined 2315 day-5 single blastocyst transfers in frozen-thawed embryo transfer (FET) cycles, yielding 489, 761, and 1103 live births, segmented by the number of cells in the day-3 embryos (<8, 8, and >8 cells, respectively). Differences in neonatal outcomes among the three groups were sought.
There was no notable correlation between the number of cells in a 3-day-old embryo and the incidence of monozygotic twins. An increase in the number of cells within the day 3 embryo was associated with a corresponding increase in the sex ratio, although this difference lacked statistical significance. There were no noteworthy discrepancies in the occurrences of preterm birth and low birth weight across the three groups. Statistically insignificant differences were found in the stillbirth and neonatal mortality rates between the three groups.
